MEDICATION INFORMATION: Medication Authorization Form

Illinois School Code mandates that there must be a written authorization, signed by health care provider, for any medication given at school.  A medication permit must be completed every school year for ALL medications. This includes Tylenol, Ibuprofen, Aleve, TUMS, cough syrup, cough drops, antibiotics, ear drops, prescription eye drops, etc... No medication will be given without a medication authorization signed by health care provider.  If you would like your student to receive any medications, please complete permit form, have provider sign it and return it to school.  NO medication will be given without a medication authorization that is completed and signed by a licensed health care provider.
ALL AUTHORIZATIONS MUST BE ENTIRELY COMPLETED BEFORE RETURNING TO SCHOOL.
